PATIENT PRIVACY

All client records at our clinics are treated confidentially and in accordance with legal and ethical requirements.

In accordance with the Australian Privacy Principles 2014 (Privacy Act 1988), the Personally Controlled Electronic Health Records Act 2012 and the Freedom of Information Act 1982, individuals have a right of access to health information held about themselves. Requests for access to client health information must be made in writing.

Evidence of identity of the person will be required. A driver’s license, pension card, student card, Medicare card or similar identification bearing a specimen signature will normally be acceptable.

Processing of the request cannot begin if any of the requested information is not supplied. A fee may be applicable for accessing information contained in client files. Fees are based on the costs involved for checking the information and preparation of the information for release (e.g. photocopying etc.).

CANCELLATION POLICY

Acu Hub Pty Ltd commits to providing the best service in a timely manner. We aim to accommodate your needs. Unfortunately, when a cancellation occurs without adequate notice, it prevents another patient from receiving treatment. We respect that your time is valuable and we appreciate that you understand ours is too. This cancellation policy is used as a way of respecting the time commitment of everyone involved.

We understand situations can arise in which you must cancel your appointment. Due to limited appointment availability, we request that you cancel or change your appointments with adequate notice. You may cancel by visiting www.acuhub.com.au and modifying your appointment or emailing [email protected].

We require a minimum of 12 hours’ notice for cancellation of appointments. If an appointment is booked for Monday, please ensure cancellation occurs no later than 5pm Sunday.

Any booking that is cancelled within the 12-hour period prior to the appointment date and time will be charged for the full appointment. Any cancellations within 48 hours but before 12 hours will incur a cancellation fee of 50% of the value of the consultation fee. Acu Hub do not issue refunds for pre-paid appointments cancelled within the 12-hour period prior to the appointment time and date.

All unpaid cancellation fees that have been charged to a client’s account must be paid prior to, or on the day of, the client’s next appointment or within 3 business days of invoice if no appointment is scheduled within 7 days. Acu Hub reserves the right to refuse any future appointment bookings should payment of cancellation fees not be made.

Cancellations must be made online at www.acuhub.com.au or via email to [email protected].

NO SHOW

Failure to present for an appointment will be considered a ‘No Show’. If this occurs, 100% of the consultation fee will be charged and must be paid prior to, or on the day of, the client’s next appointment or within 3 business days of invoice if no appointment is scheduled within 7 days.

CONFIRMATION OF APPOINTMENT

Acu Hub sends confirmation SMS – 2 days prior to your scheduled appointment time. Please take this opportunity to make any required changes to your booking. Regardless of confirmation of booking, should an appointment be cancelled within the 48-hour period prior to the scheduled appointment time, cancellation fees will still apply.

Thank you in advance for your support and understanding.
